Use these documents to help manage the health and safety of the nursery, to ensure children, staff and visitors are safe whilst at your nursery.
These documents include:
- Accident Forms – Ofsted approved accident form. It comes in two parts. Part 1: To record accidents that happen onsite. Part 2: To record children’s injuries that happened before coming to nursery.
- Activities risk assessment – assess the danger and risk of a specific activity with this activities risk assessment sheet.
- Administration of sun cream forms – record when you apply sun cream to your children so you can ensure they are playing safely in the sun with this administration of sun cream form.
- Asthma Forms – have a child that has asthma and needs to use an asthma pump? Use this form to record the medication and when the pumps are given.
- Authorisation to collect a child – use this form when a parent requests for someone else to pick up their child who isn’t on the registration form.
- Cleaning rota – a cleaning rota to allocate different tasks to your staff team.
- Detailed risk assessment – this form should be completed after a serious incident (e.g. a near miss) or serious accident happens, termly to access and evaluate incidents and/ or accidents that have occurred or when there is a regular occurrence of incidents/ accidents.
- Evaluation of risk – give this to staff to improve their assessment of risk.
- Fire drill record – use this form to record your fire drills.
- Head injury forms – use this form to record and monitor when a child has a head injury, can be given to health professionals where necessary.
- Health care plan – if your child has health issues, use this to create a plan with their parent/ carer so that they are safe at nursery.
- Incident forms – this form is to be completed by a member of staff whenever an incident occurs involving a child within the setting and filed accordingly.
- Inside Checklists – complete this checklist daily to ensure your nursery rooms are safe to use and reduce risk.
- Lockdown drill record – use this form to record your lockdown drills.
- Meal intake charts – use this form to record how much food and what food your children have eaten throughout the day
- Medication consent forms – whenever a child comes in with prescribed medicine, use this form to record how much is to be administered, where it should be stored and when it should be given. It also has a section to record when the medicine has been given.
- Nappy changing records – record your children’s nappy changes on this document
- Opening and closing checklist – use this to opening and closing checklist to ensure that your nursery is ready to open at the beginning of the day and is shut down at the end of it.
- Outdoor Checklists – complete this checklist daily to ensure your outside area is safe to use and reduce risk.
- Outings plan and headcount forms – use this form whenever you go on an outing to ensure that the outing is conducted safely.
- Physical intervention forms – if you have to use any physically touch or restrain a child you have to record the incident. Do so conveniently on this form.
- Record of child’s temperature – use this to record whenever a child has a temperature
- Risk assessments sheets – use this form to risk assess anything in the nursery, whether that be an activity, area or person.
- Room Temperature Checks – record the temperature of your rooms with this document, to ensure they are a safe temperature to use.
- Sickness monitoring form – use this to monitor a child’s sicknesses whenever a child is ill.
- Sleep check records – record your children’s sleep times with this document.
- Toy Cleaning Sheets – record how and when you clean toys with this document.
- Visitor sign in – record who attends the nursery with this simple sign in sheet.
